✦LodestoneEldercare Guide — point toward true help

The shrinking world

If caregiving keeps you from seeing friends or going out, work out what would make a visit possible: someone to cover the care, a ride, or a different time. You can ask for that help without filling out a questionnaire first.

Being cut off and being worn down are different things

The National Institute on Aging (NIA) says feeling lonely is different from having few people around. It reports that people who are isolated or lonely tend to have worse health, including depression. That is a pattern across many people. It does not predict what will happen to you.[1]

What the Zarit questionnaire can tell you

The Zarit Burden Interview is a questionnaire about how caregiving affects the caregiver. The group that manages it, Mapi Research Trust, describes a version with 22 questions. Each answer is scored 0 to 4, for a total of 0 to 88. There are also several shorter versions. A higher score means the caregiver reports a heavier load. The guide warns that the cutoff scores depend on the group of people being tested.[2]

It is not a loneliness test. Ask the professional who uses it which version they mean and how they read it. Do not treat a score above 40 as a sure sign of a breakdown for everyone. Do not assume a score decides if you can get respite. The source we checked does not support either claim.[2]

Something you can ask for without a questionnaire

This is Lodestone's own list to help you ask. It is not the Zarit questionnaire:

  • The person or activity I miss is ___.
  • What gets in the way is time, a ride, someone to cover, cost, or ___.
  • The help I am asking for is ___.
  • The day and time that would work is ___.

A request like “Could you stay with Dad on Wednesday so I can have lunch with my friend?” gives someone a clear yes-or-no to answer. Ask the person getting care what would make the plan comfortable for them, too.

If you stay upset for a long time, talk with your own doctor. For practical choices, look at respite and local help. You do not need a certain score to start asking what support is out there.

Quick answers

What does a Zarit score measure?

How heavy the caregiver says the load of caregiving is. There are several versions. The full one has 22 questions and a total score from 0 to 88. It is not a diagnosis by itself, and it does not measure loneliness. [2]

Is a score above 40 always a warning of breakdown?

No. The official guide to the questionnaire does not set one cutoff for everyone. What a score means depends on the version, the group of people, and the situation. Ask the professional who uses it how to read your result. [2]
